Business Purchase Application

Welcome and thank you for your interest in purchasing dog and cat treats from Exclusively Pet, Inc.

In order to properly set up your account, we need the following business forms completed:

(click EXCLUSIVELY PET TAX CERTIFICATE to print)

(click CREDIT CARD AGREEMENT FORM to print) (optional)

If you have problems printing the forms, you may have to download the latest version of Adobe Acrobat linked to our front page. Please note the Purchase Application and TAX CERTIFICATE must include all principal ownership information and three signatures that are highlighted in yellow. If you would like to pay by credit card, please fill out the Credit Card Agreement Form. You must still fill out the Tax Certificate if you are paying by credit card. All forms may be faxed to (414) 365-2667 or mailed to: Exclusively Pet, Inc., Attn: Credit Department - P.O. Box 245031, Milwaukee, WI 53224-9531.
